Baptismal Requirements :
	Birth Certificate - Certified True Copy (bring the original and a copy for authentication)
	Parents' Marriage Certificate - Certified True Copy (bring the original and a copy for authentication)

Adult Baptism:
	Birth Certificate - Certified True Copy (bring the original and a copy for authentication)
	Parents' Marriage Certificate - Certified True Copy (bring the original and a copy for authentication
	Attend the Catechism Seminar - 4 consecutive Mondays (Note: upon schedule of the Catechist c/o Kuya Bart de Guia, Parish Head Catechist)

Baptism for Religious Converts (from other religions to Catholic):
	Birth Certificate - Certified True Copy (bring the original and a copy for authentication)
	Parents' Marriage Certificate - Certified True Copy (bring the original and a copy for authentication
	Bring your previous Baptismal Certificate (other Religion)
	Attend the Catechism Seminar - 4 consecutive Mondays (Note: upon schedule of the Catechist c/o Kuya Bart de Guia, Parish Head Catechist)

Fees:
ORDINARY	P300 with 1 pair of Sponsor (1 Godmother, 1 Godfather) Additional Sponsor P25/head
Schedule	Every Sunday
SPECIAL	P2,000 with 1 pair of Sponsor (1 Godmother, 1 Godfather) Additional Sponsor P25/head

Confirmation

Requirements for the Sacrament of Confirmation:
	Authenticated Birth Certificate
	Authenticated Baptismal Certificate
	Authenticated Parents' Marriage Contract
	3 days Seminar scheduled by the Catechist (2 1/2 hrs per session/seminar)
	Donation of P200
	1 GodFather and 1 GodMother
	12 years and above
	Ready to receive the sacrament of Confirmation
	Approval of the parish priest

	November 26, 2005 Saturday 9:00-11:00 AM
	January 26, 2006 Thursday 9:00-11:00 AM
	March 4, 2006 Saturday 2:00-5:00 PM
